Job description
CenterWell Home Health is seeking a licensed Occupational Therapist to provide in-home rehabilitation under the Home Health Branch Administration. You will assess daily living skills, develop retraining programs and deliver therapy to restore strength, coordination and independence in patients' homes.
You will collaborate with physicians and the care team, document care observations, and participate in care coordination and discharge planning.
