Job description
Optimal Care, Inc. is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. You will assess functional abilities, design individualized treatment plans, and guide daily living activities with a focus on safety and quality of life.
This role requires a licensed OT with ACOTE accreditation, at least 1 year of experience, and willingness to travel daily within the service area. A valid driver's license and reliable transportation are essential.
