Job description
Conemaugh Home Health, part of the LHC Group within the Optum family, is seeking an Occupational Therapist to assess and address patients’ functional status, daily living activities, fine motor skills, and home safety in a home health setting.
You will collaborate with physicians and nurses to develop and implement plans of care, perform comprehensive assessments, and document CMS-relevant data to support quality care and outcomes.
