Job description
CenterWell Home Health is seeking a Home Health Occupational Therapist to assess and retrain patients in their homes, improving function, strength and independence under physician orders. You will collaborate with the care team to design retraining programs, supervise staff, document progress, and participate in care coordination and discharge planning.
