Job description
Canyon Home Care and Hospice seeks a dynamic Physical Therapist to join our Northern Colorado team. You will provide 1:1 home health PT, create individualized plans of care, and coordinate with the interdisciplinary team to meet patient needs.
You will supervise PTA and HHA as applicable, perform skilled PT procedures, and educate patients and families on home exercise programs while maintaining documentation and confidentiality per agency policy.
