Job description
Heartland Home Health is seeking a Full-Time Physical Therapist to care for patients in the Midland, MI area. You will deliver skilled PT services at patients’ homes, develop goals with physicians, and collaborate with an interdisciplinary team to restore mobility and independence.
The role requires a Master’s in Physical Therapy, a current Michigan license (or eligibility), CPR certification, and strong communication and problem-solving skills.
