Job description
UnitedHealth Group is seeking a Physical Therapist for Home Health to evaluate patient needs, develop a treatment plan, and deliver interventions to improve mobility, endurance, and comfort. You will partner with physicians and a care team to monitor progress and adjust care plans as needed.
Responsibilities include conducting assessments, developing plans of care, documenting progress, and ensuring care is delivered within state guidelines.
