Job description
Assured Home Health of Woodward, part of LHC Group within the Optum family, seeks a Physical Therapist to assess patient needs, develop a plan of care, and provide home health therapy to improve mobility and well-being. You will evaluate functional status, plan interventions, educate patients and families, and document progress while coordinating with the care team to enhance outcomes.
This role requires current PT licensure, CPR certification, and reliable transportation; a $10,000 sign-on
