Job description
Heartland Home Health, a Gentiva company, is seeking a Full-Time Physical Therapist to provide skilled rehabilitation in patients' homes from our Midland, MI branch. Deliver care based on the physician-ordered Plan of Treatment and help patients recover, restore function, and live safely at home.
