Job description
Mercy Medical Homecare seeks a compassionate Physical Therapist to join our Home Health team. The PT provides skilled therapy services in patients' homes under physician orders, develops individualized plans of care, and educates patients and caregivers to maximize independence.
You will evaluate with OASIS, adjust plans based on progress, supervise PTAs as allowed, and ensure accurate documentation and continuity of care across admissions, recertifications, and discharges.
