Job description
CenterWell Home Health is seeking a Physical Therapist to deliver home-based care, plan and administer prescribed therapy to restore function and independence for patients in their homes. Reporting to the Clinical Manager, you will assess patients, coordinate care with a multidisciplinary team, and document progress while ensuring compliance with all regulations.
This role includes a $10,000 sign-on bonus or relocation assistance, with benefits starting on day one and a company vehicle program
