Job description
CenterWell Home Health is seeking a licensed physical therapist to deliver in-home therapy, helping patients regain strength, mobility and independence in familiar settings.
You will assess impairments, implement prescribed treatment plans, supervise PTAs and home health aides, document progress, and communicate with physicians, the care team, patients and families to ensure continuity of care.
