Job description
Residential Home Health and Hospice seeks a dedicated Physical Therapist (PT) to provide skilled home health care in Clarkston and surrounding areas. You will work autonomously within an interdisciplinary team, delivering patient-centered evaluations and therapy plans to improve strength, balance, and independence.
The role requires an active Michigan PT license, at least one year of clinical experience, and a willingness to travel about 20% of your day to patient homes.
