Job description
Heartland Home Health, a Gentiva company, is seeking a full-time Physical Therapist to serve patients in West Branch, Michigan. You will deliver skilled PT services based on an individualized Plan of Treatment and collaborate with an interdisciplinary home health team.
Qualifications include a Master’s degree in Physical Therapy, Michigan DPT licensure (or eligibility), CPR certification, and strong communication skills. Relocation assistance, sign-on bonus, and fleet car program are offered.
