Job description
BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full-time home health role serving Adult and Geriatric clients across Granville and Person counties in North Carolina. The PT will perform home visits, assess mobility, balance, and wound care needs, and develop individualized care plans as part of a multidisciplinary team.
The position emphasizes independent practice, strong documentation, and collaboration with nursing staff to support patients remaining in their homes.
