Job description
Optum is seeking a Physical Therapist in Home Health to assess and treat patients, develop and adjust care plans, and monitor progress in a home health setting. You will work with the care team to improve functional status, mobility, and pain management, ensuring patient comfort and safety.
The role requires current PT licensure in the practicing state, a valid driver's license, and reliable transportation. A CPR certification is preferred; sign-on bonus is available for full-time availability.
