Job description
Provide skilled physical therapy services to patients in their homes under a licensed physical therapist’s supervision. Carry out individualized treatment plans to improve mobility, strength, balance, and independence; monitor and document patient responses; coordinate with caregivers and the care team; and support safe recovery and follow-up care.
