Job description
CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to deliver personalized care in patients' homes, helping regain strength and independence. You will work under the Physical Therapist to implement treatment plans and coordinate care with the clinical team.
The role requires a current state PT license, CPR certification, and strong geriatric care knowledge, with driving eligibility and reliable transportation to visit patients across the service area.
