Job description
As a Home Health Physical Therapist Assistant, you provide hands-on therapy in patients’ homes under a licensed PT’s plan of care. You deliver therapeutic exercises and modalities, train on mobility and use of equipment, monitor patient response, and educate patients and families. You document visits in the EMR, coordinate with the PT and broader care team, and help patients safely improve function and independence in a variety of home environments.
