nJob Description: All About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for Physical Therapist Assistants (PTAs) to help care for patients across the Upper Central North coverage of the Greater Houston area, which mainly includes Conroe, Spring, The Woodlands, and surrounding areas. If you have experience in home health and are interested in covering this area, please submit your resume. We use Kinnser software to document patient care.
nQualifications:
nGraduate from a two-year college level program approved by the American Physical Therapy Association
nCurrently licensed in the state of Texas
nTwo year of experience in Home Health preferred
nResponsibilities:
nUnderstands and adheres to established policies and procedures
nProvides physician prescribed physical therapy under a plan of care established by a PT
nImproves or minimizes residual physical disabilities of patients
nReturns the individual to optimum and productive level within patient’s capabilities
nPerforms all skilled procedures as ordered by physician and according to the plan of care established by a PT
nParticipates with all other health care personnel in patient care planning
nTakes initial history and perform initial evaluation
nConsults with PT regarding change in treatment
nInstructs patients and family/significant others in home programs and activities of daily living
nParticipates in mandatory in-service programs and presents in-service programs as assigned
nParticipates in performance improvement activities as assigned
nAttends all patient care conference as scheduled
nPrepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Agency
