Job description
Aveanna Healthcare seeks a compassionate Home Health Physical Therapist to provide one-on-one rehabilitative care in patients’ homes. Responsibilities include evaluating mobility and functional status, developing individualized treatment plans, delivering evidence-based therapy, and educating patients and caregivers on exercises and safety. You will collaborate with an interdisciplinary team, complete accurate EMR documentation, and support Aveanna’s patient-centric culture of empathy, respect, and integrity while helping patients regain independence at home.
