Job description
Addus Family Company seeks a compassionate Physical Therapist to deliver in-home rehabilitation services for seniors and individuals with disabilities. In this role, you will assess functional mobility, develop and implement individualized treatment plans, and educate clients and caregivers on exercises, safety, and adaptive strategies to support independence at home. You will collaborate closely with an interdisciplinary team, document progress in accordance with regulatory standards, and participate in ongoing training.
