Job description
Optum in Washington state is hiring a Physical Therapist for Home Health to assess functional status, mobility, endurance and pain, and to develop and implement treatment plans that improve patients' well‑being in their homes.
You will document visits, communicate progress to the care team, and ensure care aligns with state guidelines and policies. A current WA physical therapy license and reliable transportation are required; CPR certification is preferred.
