Job description
Advocate Health At Home seeks a licensed Physical Therapist to manage a patient caseload in the home setting across Charlotte, NC. You will collect and analyze health data, develop and implement plans of care, and coordinate with physicians to ensure timely, high-quality rehabilitation services.
Responsibilities include ongoing documentation, supervision of PT assistants and home health aides, and adhering to state licensure and safety guidelines while traveling to patients' residences.
