Job description
EvergreenHealth in Redmond is seeking a licensed Physical Therapist to provide skilled home health care and coordinate with patients, families and the care team to achieve functional goals.
You will assess, plan, implement and evaluate care plans, adjust as needed, educate patients and caregivers, and communicate changes to physicians. Washington state licensure, current BLS, and reliable transportation are required; home health experience is preferred.
