Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to join our caring community. You will report to the Clinical Manager and help patients regain strength, mobility and independence from the comfort of their homes.
You will plan and administer prescribed therapy, assess progress, develop rehabilitation programs, supervise PTAs and aides, document evaluations, and report progress to the care team and family.
