Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, evaluate patients, and design personalized rehabilitation plans to maximize function and independence. You will document progress, coordinate with physicians and care teams, and guide caregivers to ensure quality care.
Requires current physical therapy licensure, CPR certification, and ability to work in patients' homes; driving and home visits are expected, with compensation reflecting full-time hours.
