Home Health Physical Therapist - Evans, CO
Looking to make an impact for patients and families in your community? HealthCare Support is seeking a Home Health Physical Therapist in Evans, CO with a 4.5 STAR-rated healthcare provider. As a Home Health Physical Therapist, you’ll have the opportunity to provide personalized, one-on-one care to patients in their homes.
Shift: Monday - Friday, 8a-5p
Compensation: $101,000 - $112,0000 annually (DOE)

Daily Responsibilities
Deliver personalized in-home physical therapy, helping patients regain mobility, strength, balance, and independence while educating families and tracking progress in collaboration with the care team
Required Qualifications
1 year as a Physical Therapist
Master's Degree in Physical Therapy
Licensed Physical Therapist in the state of practice
Licensed Driver with reliable transportation
