Job description
Safe Life Home Health Care, LLC is seeking a licensed Physical Therapist to deliver home health PT services in DeKalb County. You will supervise PTAs and implement plans of care per physician orders.
Requirements include an IL license, at least one year of home health experience, and access to a car with valid insurance. You will conduct timely evaluations, maintain accurate notes, and coordinate with the care team and physicians.
