Job description
Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide in-home therapy, improve mobility, and support daily living activities for patients. You will administer skilled care per physician orders and collaborate with family and care teams to implement the established plan of care.
Responsibilities include patient education, safety coaching, and coordination with the Branch Director. A valid state license and reliable transportation are required in this role.
