Job description
AdventHealth in Altamonte Springs, Florida is seeking a licensed Physical Therapist for home health care. You will develop patient-specific plans, reassess progress, and document outcomes. You will coordinate care, prioritize caseloads, and guide patients and families in home programs.
The role requires at least 1+ year of clinical PT experience, a valid PT license, and willingness to work within Medicare-certified home health settings. COSC is preferred but not required.
