JAG Physical Therapy seeks an At Home Physical Therapist to provide one-on-one, evidence-based rehab services to patients in their homes across New Jersey. You will evaluate and treat orthopedic and post-surgical conditions, develop personalized care plans, document in EMR, and collaborate with physicians and the JAG team.
