Job description
VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across Wisconsin. You will evaluate mobility, design and implement personalized treatment plans, and collaborate with the care team to optimize recovery and independence.
You will supervise PTAs as allowed by state guidelines and educate patients and caregivers to sustain progress. A PT license and CPR certification are required, with 1–2 years of home health experience preferred.
