Job description
Compassus is seeking a Home Health Physical Therapist to plan, implement, and evaluate patient care plans across age groups. You will collaborate with an interdisciplinary team to provide coordinated physical therapy services, tailored to medical referrals and ongoing assessments.
Responsibilities include conducting initial and ongoing assessments, developing plans of care with physicians, and educating patients and families on transfer techniques and safe equipment use.
