Job description
CenterWell Home Health is seeking a dedicated Physical Therapist to help patients regain strength, mobility and independence in their own homes. Reporting to the Clinical Manager, you will design and implement PT plans focused on functional improvement and rehab goals.
You will assess patient needs, document progress, and collaborate with PTAs and Home Health Aides to deliver clear care instructions and ensuring quality care across the patient team.
