Job description
Ascension at Home together with Compassus seeks a Home Health Physical Therapist to deliver patient-centered physical therapy in home settings. You will plan, implement, and evaluate therapy plans based on referrals and evaluations; collaborate with the interdisciplinary team to optimize patient outcomes.
Qualified candidates hold a BS in Physical Therapy, possess an active state license, and have CPR certification.
