Job description
Enhabit Home Health & Hospice is seeking aLicensed Physical Therapist to provide skilled therapy in patients' homes. You will assess, plan and administer treatment to improve mobility, independence and quality of life, while coordinating with physicians and family members to support ongoing care.
The role emphasizes patient education, safety during daily activities, and program adherence across a range of home health services. Strong driving means and state licensure are required.
