Job description
Atmore Community Home Care, part of LHC Group within Optum, is hiring a Physical Therapist in Home Health to assess patient needs, determine treatment plans, perform interventions, and evaluate progress. You will work with physicians and the health care team, educate patients and families, and document outcomes.
Requires current PT licensure in the state, a valid driver's license with reliable transportation, and CPR certification preferred within 90 days.
