Job description
Mountaineer Home Care, part of the LHC Group family, seeks a licensed Physical Therapist for home health services. You will assess functional status, plan care, and provide interventions to improve patient well-being while coordinating with the care team.
Responsibilities include developing plans of care, documenting progress, and ensuring compliance with state guidelines. A driv er's license and CPR certification are required or obtainable soon after hire.
