Job description
Elite Home Health of Fordyce, part of LHC Group and Opptum family, seeks a Physical Therapist to evaluate and manage patient care in home health. You will assess functional status, plan and implement treatment, and monitor progress in collaboration with the care team.
Responsibilities include documenting visits, ensuring adherence to state practice standards, and educating patients and families. Requires state PT licensure and reliable transportation; CPR preferred.
