Job description
Home Health Physical Therapist in Marion, OH
Put your expertise to work helping patients achieve greater independence by delivering compassionate, evidence-based rehabilitative care in a home health setting. This full-time, 13-week contract position offers the chance to make a meaningful difference as you work autonomously with diverse adult clients.
Schedule/Hours:
Monday to Friday, 8:00 AM 5:00 PM
Caseload: Expect approximately 25-32 points per week (5-7 visits/day, variable by visit type)
Field-based, traveling daily to patient homes within an assigned territory
Key Responsibilities:
Conduct comprehensive evaluations and complete OASIS assessments as required
Develop and implement customized physical therapy plans of care tailored to patients individual needs
Deliver skilled interventions, including therapeutic exercise, gait and transfer training, fall prevention, and neuromuscular re-education
Assess progress, adjust treatments, and communicate effectively with physicians and interdisciplinary team members
Instruct patients and caregivers on exercises, home safety, assistive devices, and home exercise programs
Accurately document patient visits using Home Care Home Base (HCHB)
Collaborate with nurses, therapists, social workers, and home health aides for coordinated, high-quality care
Attend interdisciplinary meetings and participate in ongoing professional development
Qualifications:
Active, unrestricted Physical Therapist (PT) license in the state
Minimum 1 year of recent home health PT experience required
OASIS assessment experience required; Home Care Home Base (HCHB) proficiency preferred
Excellent clinical assessment, organizational, and communication skills
Reliable transportation, valid drivers license, and proof of auto insurance
Comfortable working independently and managing a flexible, dynamic schedule

$2,299 - $2,432/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
