Home Health Physical Therapist in Waukee, IA
Bring your clinical expertise and compassion to the forefront as a dedicated Home Health Physical Therapist, providing vital care to patients in their residences. This contract opportunity is perfect for a skilled PT who thrives in a dynamic, patient-driven environment and values autonomy in delivering evidence-based therapy.
Schedule:
Monday to Friday, 8:00 AM 5:00 PM
Responsibilities:
Complete thorough initial and ongoing physical therapy evaluations, including OASIS assessments for various episodes of care.
Develop individualized care plans targeting improved mobility, strength, functional independence, and safety.
Deliver hands-on skilled interventions such as gait training, neuromuscular re-education, transfer training, therapeutic exercise, and fall prevention.
Closely monitor patient progress, adjust plans as indicated, and communicate vital updates to physicians and the care team.
Educate patients and caregivers on exercise programs, safety, use of assistive devices, and strategies for functional improvement.
Document care promptly and accurately using Home Care Home Base (HCHB).
Collaborate with interdisciplinary team membersRNs, OTs, SLPs, medical social workers, and HHAsto coordinate holistic care.
Attend case conferences and pursue continuing education for ongoing professional growth.
Adhere to agency policies, Medicare home health standards, and best practice protocols.
Desired Qualifications:
Active Physical Therapist (PT) license (state of practice), in good standing.
Minimum one year of recent field home health PT experience.
OASIS proficiency required; Home Care Home Base (HCHB) familiarity strongly preferred.
Outstanding decision-making, assessment, organization, and communication skills.
Reliable transportation, current drivers license, and valid auto insurance.
Professional independence combined with a collaborative spirit.

$2,299 - $2,432/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
