Job description
Addus Family company seeks a compassionate Physical Therapist to deliver evidence-based rehabilitation in home and community settings. You will assess functional mobility, develop individualized treatment plans, and provide hands-on therapy to help seniors and individuals with disabilities maintain safety and independence at home. Collaborating with an interdisciplinary team, you will educate clients and caregivers, document outcomes, and adjust plans to optimize progress.
