Job description
Wellstar Health System in Georgia is looking for a Physical Therapist to provide and supervise home health care for about 25–35 patients. You will coordinate with RNs, therapists, social workers, and physicians to develop and implement the plan of care.
Responsibilities include guiding visits, adjusting the care plan per physician orders, and leading the care team to achieve positive patient outcomes in a supportive environment.
