Job description
As a Home Health Physical Therapist, you provide rehabilitation services to patients in their homes, evaluating mobility and functional needs and developing individualized plans of care under physician orders. You manage a caseload, deliver treatments, document visits and outcomes, and coordinate with an interdisciplinary team. You also identify risks such as falls or rehospitalization and help ensure safe, goal-directed care for diverse adult and pediatric patients in varied home environments.
