Job description
Role: Physical Therapist / PT, Home Health
Key Responsibilities
Perform patient evaluations and develop evidence-based treatment plans
Provide therapeutic interventions to improve mobility, function, and safety
Collaborate with physicians, nurses, and caregivers to coordinate care
Document patient care accurately in EMR systems
Educate patients and families on treatment and safety techniques
Qualifications & Skills
Licensure as a Physical Therapist
Strong clinical skills in home-based rehabilitative care
Excellent communication and collaboration abilities
