Home Health Physical Therapist - Per Diem
Please note that this is a per diem position that is paid on a per visit basis. It is ideal for a PT looking to supplement their income, pick up additional hours and does not want a set shift type of schedule. You will be expected to visit patients in their homes with in Santa Clara, San Mateo and/or Alameda counties
InTouch Home Care Inc., is currently seeking a self‑motivated Physical Therapist to deliver exceptional care and is enthusiastic about working with adults of all ages. The Home Health PT is responsible for the assessment and evaluation of the patient, and works to promote the patient's ability to move, reduce pain, restore function, and prevent disability. Based on their assessment and evaluation, the Physical Therapist determines a treatment plan, executes interventions aimed at improving and enhancing the patient's well‑being, and monitors the patient's progress effectively.

Proven sound clinical reasoning in treating and planning, implementation and monitoring patient progress.
Ability to recognize when patient protocols or treatment plans need modification to meet appropriate patient needs and report to the therapist.
ESSENTIAL JOB FUNCTIONS
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
Regularly re‑evaluates patient rehab needs.
Initiates a care plan and makes necessary revisions as patient status and needs change.
Care plan establishes measurable goals and outcomes using standardized assessment measures by which to create progress benchmarks. Includes the patient and family in planning process.
Initiates appropriate preventive and rehabilitative procedures.
Counsels the patient and family in meeting PT needs
Provides health care instructions to the patient as appropriate per assessment and plan of care.
Completes documentation of visits within 24 to 48 hours of seeing patient to ensure reimbursement.
COMMUNICATION
Maintains open communication via the agency communication method (HIPAA compliant messaging app) with regard to visits schedule, progress, and any issues that arise through the duration of patient care.
Communicates with interdisciplinary team, agency staff, patient's primary care physician and their family to coordinate patient's care plan.
Communicates any changes in patient's condition to the patient's physician and obtains/receives and logs any physician orders as required.
ADDITIONAL DUTIES
May be expected to supervise any aide staff on a biweekly basis
POSITION QUALIFICATIONS
PT license and registration by the state.
Completion of a PT curriculum approved by the American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or the Council on Medical Education of the American Medical Association.
CPR certification
Excellent observation, verbal and written communication skills, problem solving skills, PT skills per competency checklist
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
