Home Health Physical Therapist – Per Diem
Job Location: Kanawha and Putnam counties, WV
Position Summary:
Provides physical therapy services as ordered by the physician for Home Health of WV.
The Physical Therapist (PT) provides consultation to the clinical team.
Performs other related duties as required.
Position Qualifications:
Graduate from an approved school of physical therapy.
Current unrestricted WV license to practice as a Physical therapist.
Physical Demands / Work Environment:
Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
Frequent standing and walking for extended periods (8–12 hour per shift).
Frequent bending, stooping, kneeling, crouching, and reaching.
Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
Visual acuity to read medication labels, physician orders, and monitors.
