Job description
Northern Arizona Home Health, part of the LHC Group and within the Optum family, is seeking a Physical Therapist for home health in the Prescott, AZ area. You will evaluate patients, develop treatment plans, and provide interventions to improve function and well-being.
Primary responsibilities include evaluating functional status, collaborating on plan of care, and documenting progress. Eligibility requires PT licensure in the state and a valid driver's license; CPR is preferred.
