Job description
We are seeking a Home Health Physical Therapist to provide personalized care in patients’ homes, focusing on improving mobility, strength, and independence.
Responsibilities include planning and administering prescribed physical therapy treatments, assessing and documenting patients’ physical abilities, developing rehabilitation programs aligned with physician plans, supervising PTAs and aides, and reporting patient progress to the care team and family.
Qualifications require an unrestricted Physical Therapy license , CPR certification , and experience working with healthcare professionals.
Preferred candidates have at least one year of PT experience.
The role involves patient-facing responsibilities, requires a valid driver’s license, and adherence to TB screening and driver safety programs.
Compensation ranges from $93,000 to $128,000 annually, with benefits supporting whole-person well-being.
